APPLICATION PROCEDURE 1. The student is to neatly fill in all data requested on the application blank.
a. Attach a snapshot or small photo
b. Enclose a $10 application fee
2. Request an official transcript from the last school you attended be sent to us.
3. Get three people to fill out a Recommendation Form, and have the individual mail them directly to
us (please do not include them with your application).
a. Principal or Teacher
b. Pastor
c. Friend (adult) who knows you well and is not a relative
4. Have a doctor fill out a Physical Examination and Immunization form. 5. Get a copy of your Birth Certificate and your Social Security Card. 6. When all of the following items have reached Laurelbrook Academy, your application will be processed: a. Signed application with $10.00 and photo
b. Transcript from last school attended c. Three (3) reference completed by a pastor, school official, or family friend (someone who knows you well)
d. Physical Examination and Immunization form completed by a physician
e. Copy of your Birth Certificate
f. Copy of Social Security Card
